Parents, if your baby is between the age of 1wk and 6 months, you can register by mail, drop off your application to Perfect Image Cosmetic Gynecology or apply here online.

If completing the entry online, the following must also be mailed to Perfect Image Cosmetic Gynecology to complete the registration:

• 4" x 6" photo
• Birth Certificate (photocopy)
